SHICK Training Registration Form

Register for your selected course below. Room capacity for In-person training is limited and registration will be closed when courses have reached capacity. Each training date has a minimum attendee requirement of 3. If this minimum is not reached, registrants will be notified to choose a different training date.

Please review all fields for accuracy, as a registration confirmation will be sent to the email address provided.

Both Initial Training sessions are scheduled from 9:00 a.m. to 3:00 p.m.
Update Training sessions are scheduled from 9:00 a.m. to 4:00 p.m.

Please select one type of training, either Initial training or Update training. Initial training requires registration in BOTH Session 1 and Session 2. If you accidently select the wrong type of training, use the last choice to indicate your cancellation of that selection.

Experienced counselors may choose to attend Initial trainings for review or updating skills. New applicants may choose to attend an Update training after attending the Initial training sessions for additional skills and networking.

New Applicants must attend both sessions of the Initial training to certify. Experienced counselors must attend Update training to re-certify.
New counselors
Register for Session 1 Initial Training - Medicare Basics. Training is from 9:00 a.m. to 3:00 p.m.

Session 1 - Initial Training - Medicare Basics - 9:00 a.m. - 3:00 p.m. - 5 hours

New counselors
Register for Session 2 Initial Training - Program Basics. Training is from 9:00 a.m. to 3:00 p.m.

Session 2 - Initial Training - Program Basics - 9:00 a.m. - 3:00 p.m. - 5 hours

Experienced counselors - register for Session 3 Update Training. Training is from 9:00 a.m. to 4:00 p.m.

Dates and/or locations are subject to change or cancellation if there is inadequate registration.
Session 3 - SHICK Update Training - 9:00 a.m. - 4:00 p.m. - 6 hours

You will receive a confirmation email from the State SHICK Office once your registration request has been received in our office. Please note that the times of training are listed on this form and on your confirmation. Links to join the Videoconference and/or to access training materials will be sent later.
